The required forms for the proposal approval process to undertake clinical research in conjunction with Global Orthopaedic Technology can be accessed below. Simply click on 'Clinical Research Approval forms for Download'.  Once the completed paperwork is received by Global Orthopaedic Technology, a member of the Clinical Research team (see below), will contact you to verify the information received.  A letter, acknowledging the study proposal, will then be sent to you.  It will also let you know the date of the next Clinical Advisory Committee meeting, where the proposal will be assessed.  After the meeting a letter will be sent to you within two weeks providing the outcome of the meeting regarding your study proposal.
